Summary of Position We are seeking a physician trained in
ophthalmology with subspecialty training in medical retina to join
us in pursuit of high-quality patient care to provide Ophthalmology
services including but not limited to diagnosis, treatment,
coordination of care, preventive care and health maintenance. The
Ideal Candidate: One to three years of ophthalmology clinical
experience in an outpatient setting. Ability to treat and diagnose
a wide variety of vitro-retinal conditions and improve patient
outcomes. Demonstrate competency in diagnosis and treatment to
assure high standards of patient care and safety are maintained.

Excellent communication skills. Interest in population health
management and value-based care. Principal Accountabilities Review
patient medical history, reports and examination results. Perform
retinal examination, and order/interpret tests (OCT, photos,
fluorescein angiograms, and other tests as approriate). Evaluate,
diagnose, and provide appropriate treatment and patient care for a
wide variety of vitro-retinal conditions. Treat retinal diseases,
such as diabetic retinopathy, vascular occlusions, and macular
degeneration, Perform office-based procedures, including:
Intravitreal injections. Focal and pan-retinal photocoagulation.
Laser of retinal tears Prescribe medications and formulate ongoing
treatment and disease management plans. Recommend suitable
management for the patient, including but not limited to,
preventive health recommendations and habits. Collaborate with
primary care providers and internal and external optometrists and
ophthalmologists, including internal virtual consults. Provide
relationship-centered care, taking into account the patient s
psychosocial and physical needs. Maintain complete and accurate
medical records. Communicate effectively with the clinical team to
ensure total delivery of quality care. Participate in medical staff
educational programs and meetings. Qualifications Education,
Training, Licenses, Certifications Doctor of Medicine (MD), Doctor
of Osteopathic Medicine (DO), or foreign equivalent Completion of
three year post graduate medical training from a U.S. accredited
residency program in ophthalmology. Completion of a medical or
medical-surgical retina fellowship, preferably one registered with
AUPO-FC Relevant Work Experience, Knowledge, Skills, and Abilities
Seeking both experienced 2 years or above, and newly trained
providers Board Eligible/Board Certified in Ophthalmology Must
possess current, unrestricted New York State license or be eligible
for licensure Must possess active Drug Enforcement Agency (DEA)
registration or be eligible for registration Must possess active
Medicare/Medicaid Enrollment or be eligible to enroll Basic Life
Support (BLS) (Required) or Advanced Cardiac Life Support (ACLS)
strongly preferred Must possess strong communication skills both
oral and written Comfortable with Electronic Medical Record (EMR)
system and Microsoft Office
