Geriatrics opening in Milford, Connecticut. This and other nurse

Geriatric Physician will provide Geriatric Assessments and
Home-Based Primary Care in an around the community of Milford CT.
Some Inpatient consultative care to enhance continuity for patients
is anticipated. Works with team of 7 Geriatricians and 10 APPs in a
growing program that supports an Age Friendly Hospital and Health
system.IM or FM, fellowship in Geriatric Medicine preferredBoard
certified/eligible in Geriatric MedicineCall is 1 in 8 weekends and
1 in 8 weeknights with some of those as back-up call to APP (so
rarely get a call)1 weekend a year medical resident team
attendingClinical faculty appointment at Yale school of Medicine if
desired.
