Job Description:
DescriptionThe Community Integration Services Program Manager is
a professional who actively supports the overall philosophy and
mission statement of CHD including CHD/Adult Mental Health. They
also maintains a high level of commitment to program staff. The CIS
Program Manager provides support, training and supervision to
Direct Care Staff as they assist individuals with co-occurring
disorders of mental illness and chemical addiction in their efforts
to live independently in the community. The CIS Manager works
within the program's model, establishing open communication,
support, and accountability among co-workers and staff. The CIS
Manager is responsible for the training and supervision of all
Full, Part-Time and Direct Care Staff. The CIS Manager is
accountable to the Assistant Program Director, who provides
supervision and training. Overall responsibility for 24-hour
staffed group home located in New Milford, VT. Supervise direct
care staff, implement client treatment plans, participate in
on-call system, oversee clinical documentation and the utilization
of an Electronic Health Record system; serve as a liaison to
external providers and the community at-large. Requirements BA/BS
Degree in Mental Health related field preferred; HS Diploma or GED
required; one year supervisory experience required; two years in a
human service organization preferred; two years of experience
working with adults with severe and prolonged mental illness;
and/or two years experience working with people in recovery.Equal
